Utah Healthcare Executives (UHE), in collaboration with The American College of Healthcare Executives (ACHE), is organizing a comprehensive program to address this shift, focusing on the overarching theme of population health.

Understanding the Paradigm Shift

As we pivot from the traditional volume-centric models, healthcare organizations bear a heightened responsibility. It’s no longer just about treatments and hospital visits. It’s about:

  • Resource Allocation: Ensuring adequate resources align with the needs of a given population.
  • Community Health: Proactively working to keep patients out of acute healthcare settings, aiming instead for community-based care.
  • Equitable Care: Coordinating care that addresses social determinants of health with an emphasis on equity.

The Role of Pioneering Models

Several trailblazing organizations have stepped up to the challenge, creating models like Accountable Care Organizations. These models obligate them to take full responsibility for the beneficiaries under their care. With an increase in the acuity of care, the focus now shifts from merely treating patients to ensuring their overall well-being in the community.

Challenges in Population Health Management

The task is monumental. How do healthcare entities efficiently reduce costs while simultaneously enhancing the quality and accessibility of care, especially in underserved communities? These communities often lack the necessary resources and knowledge for self-care, preventing acute health crises, and managing chronic diseases. This hurdle remains a significant challenge in the widespread adoption of population health strategies.
